Permission Slip

Pack 3794

Dear Parents,

Cub Scout Pack 3794 plans several outings throughout the scouting year for the boys in our Pack and will sometimes include the entire family. We may use public transportation to participate in these outings. We also may take pictures and place them in the local newspaper, Scout Shout (our newsletter) or eventually on our website. Per BSA regulations, we will never use the boy’s last name in any publication. We ask for your signature on this form to grant your permission to participate in the outings, ride on public transportation and print pictures to promote our Pack. This permission slip will be signed annually in spring.

I hereby give my permission for this child and his family to participate in outings with Cub Scout Pack 3794 and ride on public transportation when necessary. I further agree to hold the above named unit and its leaders blameless for any accidents that might occur during this outing except for clear acts of negligence or non-adherence to BSA policies and guidelines.

Permission to promote our Pack

I give permission for members of Pack 3794 to use my child mentioned above and his family in the following ways to promote this Pack:

· Take pictures at Pack or Den related events and publish them in the following (check all that apply):

Local newspaper

Scout Shout (Pack newsletter)

Website for the Pack (sometime in the future)

· Be mentioned in newspaper articles by first name only (Boy Scout of America policy)

· Participate in radio promotions
